Many thing's <g> are "common practice", like using "loose" when people
mean "lose", mixing "they're", "their", "there" etc., using "I would of
thought so" if they mean "I would have thought so", the use of "between
you and I" instead of "between you and me", etc.

That does not mean they are correct.

This one drives me /nuts/. I hear "between he and I" all the time.
Infuriating.

Naw,... those are just spelling errors. Not the same thing at all.

Aaaargh. A mistake a Dutch or German speaker of English would never
make, since that sounds horrible (and sooooo wrong) to us.

I'm not sure. I'd assume that most people knew the difference between
those three, and yet some use the same wrong one all the time. I'd call
that a grammatical error. So are errors of the kind "I would of done
that".

So are most 's after plurals. If it is possible to do that without
creating something ambiguous, the apostrophe should be omitted.

Well, it /should/ sound horrible to English speakers as well.

Here's why it happens: Kids always say "Me and Fred went to the
store". And their 4th Grade teacher always says "No, it's 'Fred and I
went to the store.". And people here that so much that they start
thinking it is never right to say "Fred and Me".

I know. It is called hypercorrection.

You can't really presume that. For example, nearly every post I write
on these forums I compose using voice dictation. The voice dictation
software often makes mistakes, and generally I catch them. But
sometimes I don't. So it's very easy for me to make that sort of error
even though I know the difference between them.

Oh cool. Sure, software won't readily know the difference. And yet, I
assume that most people making these mistakes do not use voice
dictation and can't blame their software.

Also note that I said "most people", not "all people".
